At least Viz had gotten better over the years. I guess.
Re: D.Gray-man volume 3 (released by Viz)

Its one thing to change the spelling of character names between volumes, Viz, but two different spellings on one page? Looks like someone completely skimmed past this page while proofreading. XD Me, I prefer spelling her name "Linali" over "Lenalee" since that's what I was used to seeing on scanlations. Just like I prefer "Raito" to "Light" in Death Note.
Speaking of proofreading, nothing takes me out of a story more than misspelled words. Like this panel from the sixth volume of Monster:

Looks like Viz needs a spellchecker. -_-; What irritated me the most is that the scene prior to this panel was full of such tension--I was so absorbed in it I was holding my breath in! Then that simple word completely ruined the mood for me. ARGH.

I've been reading Hunter x Hunter lately, and the little "about the author" section mentions that Yoshihiro Togashi is married to fellow mangaka, Naoko Takeuchi. I kept on thinking to myself, "Naoko Takeuchi, Naoko Takeuchi. Why does this name sound so familiar?!" And then I realized, Oh! This is the creator of Sailor Moon! [feels silly]

Also, while playing Okami yesterday I nearly threw my controller to the floor in a fit of gamers' rage after I couldn't get past a Blockhead (I wasted, like, 20 minutes on the damn enemy) in the dungeon I'm currently in. So I turned off the game before I started ripping out my hair in frustration and when I went to bed I suddenly remembered how those things were destroyed. And its so simple! Argh, I hate it when this happens.
